Medical:

Sharicom Home Health provides home health services in the Commonwealth of Kentucky. Home Health Care Nursing and Medical Surgical Services. RN's, LPN's, and CNA's offer health assessments, education and care planning in conjunction with the patient's physician and provide: 

Home Healthcare

• Medication management and education
• Chronic disease management 
• Infusion therapy
• Wound and ostomy management 
• Cardiac monitoring 
• Mental and behavioral healthcare
• Home health aide intermittent personal care support

Medical Surgical Nursing 

• Primary care
• Palliative care
• Transitional care

Social Work

• Medical social workers collaborate with the patient, their family, their healthcare team, and the community to provide support and ensure the patient is set-up for success wherever they call home. 
• Connects patient with community resources

Private Duty Services

Registered nurses, licensed practical nurses, state-tested nursing assistants, and home care aides provide the following services by the hour, by the shift or around the clock:

• Skilled nursing for medically complex needs
• Medication management
• Assistance with walking and/or transferring
• Assistance with exercise
• Personal care and grooming
• Meal planning, preparation and assistance with feeding
• Light housekeeping, laundry and linen change
• Companionship and respite care
• Errands, including grocery shopping and picking up medications
• Transportation and accompaniment for doctor appointments or social gatherings 
• Specialized care for Alzheimer's/dementia and mobility disorders

Non-Medical:

We provide non-medical companion, homemaker, and personal care services to seniors, Alzheimers', those recovering from surgery, new and expectant mothers, and others.
